How to Apply for a Library Card

​

The Cahokia Public Library District requires applicants to provide two valid proofs of address before issuing their free Library card.

 

One of these proofs must be a photo ID. This information must be presented prior to filling out a library card application.

 

On this application, applicants will be asked to provide full name (First, Middle, last), address, phone number, ID Number, and date of birth.

 

Applicants may also provide an email address for the library to contact them via the internet.

You can print and fill out the applications before arrival or when you get to the library.

ACCEPTED PROOFS OF RESIDENCY

ADULTS

​

1. Valid Photo ID:

  • Driver’s license or State ID

(If the address on the ID doesn’t match the Address on the items below we require 2 of the items below.)

-AND-

​

2. One of the following (unless ID doesn't match address as specified above):

  • Occupancy Permit or Lease Agreement

  • Utility Bill (Dated within the last 30 days)

  • Mortgage Statement

  • Tax Bill

  • Paycheck with address (Dated within the last 30 days)

MINORS

​

1. Parents must bring a Valid Photo ID:

  • Driver’s license or State ID

(If the address on the ID doesn’t match the Address on the items below we require 2 of the items below.)

 

-AND-

 

2. One of the following (unless ID doesn't match address as specified above):

  • Occupancy Permit or Lease Agreement with the child’s/Children’s name(s) on it.

  • Medical card with the child’s/Children’s name(s) and address on it.

  • An official letter from the school with the child’s name and address on it.

 

 

 

We understand that college students and young adults may not have a utility bill in their name or may be living with their parents/ guardian.

These applicants may use official statements from their college or cellular phone provider.
